His束内伝導遅延により再伝達するgap現象と交代性心室内変行伝導を認めたsick sinus syndromeの1例

A case of sick sinus syndrome with gap phenomenon due to conduction delay in the His bundle and alternating intraventricular aberrant conduction

　房室伝導におけるgap現象は，心房早期興奮の連結期を次第に短くすると，ある連結期で心室に伝達されなくなるが，さらに連結期を短くすると，再び心室に伝達される現象をいう。

　今回，著者らはHis束内における伝導遅延により，刺激が再び心室に伝達されるgap現象を認め，しかも，連結期の短縮によりQRS波形は右脚ブロック型の心室内変行伝導を示し，再伝達を示す連結期においては左脚ブロック型の心室内変行伝導を示した洞機能不全症候群の1例を経験した。本例におけるこれらの出現機序について，His束内における縦解離の存在が示唆されたので，電気生理学的検討成績と共に考察を加えて報告する。

A case of sick sinus syndrome with gap pheno-menon due to conduction delay in the His Bundle and alternating intraventricular aberrant conduction was reported. A complete RBBB pattern appeared at coupling interval (CI) between 425~390 msec. Further shortening of the CI from 385 to 365 msec resulted in HV block. At CI between 360 and 315 msec, a stimulus reconducted to the ventricle with complete LBBB pattern due to conduction delay (H₂-H^2'=80 msec) in the His bundle.

In this case, the longitudinal dissociation in the His bundle was suggested on the mechanisms of this gap and alternating intraventricular aber-rant conduction.
