心室中隔欠損症における三尖弁限局性細菌性心内膜炎

Isolated tricuspid valvular insufficiency due to infective endocarditis in ventricular septal defect

　右心系内膜炎の三尖弁限局型は，1868年Duroziezによりはじめて報告された。その後，ヘロインおよびアルコール中毒者，器質的心疾患において，数十例の報告をみるにとどまる^1〜7）。最近われわれは，心室中隔欠損症（以下VSD）に感染性心内膜炎を合併し，その結果三尖弁閉鎖不全症（以下TI）を来した2例を経験したので，文献的考察を加えて報告する。

We reported two cases of in tricuspid valve endocarditis with ventricular septal defect. Both were surgically treated; one with annuloplasty of De Vega' Maneuver and another with Hancock prosthetic valve replacement. In ventricular septal defect, the jet lesions including the tri-cuspid valve were involved by endocarditis.

But it is not easily clinically diagnosed because changes on the murmur are not clear. On echo-cardiographic study, right ventricular enlarge-ment, paradoxical septal movement and fluttering of the tricuspid valve were seen in both cases. This is an important implication of non-invasive diagnosis.
