若年性肝硬変を伴った多発性小肺動静脈瘻の1例

A case report of multiple small arteriovenous fistula of the lung associated with juvenile hepatic cirrhosis

　肺動静脈瘻は先天性中胚葉性血管形成不全による肺動静脈間の短絡形成であるが，時に全身の著明な毛細血管拡張を伴いRendu-Osler-Weber病の肺における部分症としてもみられる。病型としては単発性ないしは数個の病変があっても胸部X線像や肉眼所見で明らかにそれと認められるものから全肺野にわたり多くの微細な病変を呈するものまで種々の病型が認められている。本症の報告は1897年Churton^1）が剖検例で行ったのが最初であり，1942年Hepburn^2）らが右肺切除術による軽快例を報告したのが外科治療例の最初である。その後多くの報告があるが1959年にはBosher^3）らが350例を，1962年にはMoyer^4）らが250例を集計して統計的に検討している。本邦では1955年和田^5）の報告にはじまり，1973年田村^6）らが本邦の46例を集計して詳細に検討している。これらによると本症例の多くは単発性もしくは多発性の腫瘤状のものが占めており，びまん性に病変の存在する多発性小肺動静脈瘻の報告はきわめて少なくその病因・病態については未だ不明な点が少なくない。

A case of pulmonary A-V fistula associated with juvenile hepatic cirrhosis was presented. The patient, 6 year-old boy, was first notified to have moderate degree of cyanosis, digital clubbing and systemic multiple teleangiectasia at the age of 3 years.

Cardiac catheterization and cardio-angiography denied the congenital heart disease, but the pulmonary arteriovenous fistula in right lower lobe was suspected.
