Poor R wave Progression (PRWP) is an often used electrocardiographic criterion of old antero-septal myocardial infarction. The purpose of this study is to evaluate the electrocardiographic and vectorcardiographic differential diagnosis of old anteroseptal MI with PRWP utilizing the corresponding coronary arteriography and left ventriculography.

The ECGs of ninety-five of 1200 consecutive patients who underwent the coronary arterio-grapy and left ventriculography showed PRWP. PRWP was termed present when the absolute magnitude of the initial R wave less than 1.5 mm in V2 or less than 3.0 mm in V3. Five groups were identified in those patients, that is, old anteroseptal MI (MI; 29 cases), left ven-tricular hypertrophy (LVH; 19 cases), significant coronary stenosis of left descending artery (Stenosis; 15 cases), idiopathic cardiomyopathy (ICM; 14 cases) and normal subjects (Normal; 18 cases).
