Conduction charcteristics in the ischemic myocardium were studied using close-bipolar plunge wire electrodes impaled at multiple sites of the endocardial and epicardial surfaces. Acute myocardial ischemia was introduced by ligating the proximal left anterior descending coronary artery in 41 open-chest dogs. During first few minutes (2-7 min), a composite or single bipolar electrogram from the central ischemic zone epicardium (IZepi) displays a marked fragment-ation into multiple irregular spikes, and a total IZepi duration increased progressively.

Initiation of ventricular arrhythmias (VA) was invariably preceded by a marked fragmention extending into diastole and bridging the interval between a preceding sinus best and a first beat of VA. The maximal IZepi delay during first 10 minutes was significantly longer in 25 dogs with VA than in 16 dogs without VA.
