ヒス束内ブロックを示した1症例—近接双極多極カテーテル電極を用いて

A case of intra His bundle block--using multiple close bipolar electrode catheter

　最近我々は，II度房室ブロックから完全房室ブロックへと移行した1症例を経験し，我々の新しく考案試作した近接双極多極カテーテル電極を用いて，His束心電図（HBE）の測定を試みたところ，分裂したHis束電位が認められ，その伝導様式が，His束のproximal portionでtransverse blockを起こしているのか，またはproximalおよびdistalでblockされたfunctional longitudinal dissociationを起こしている可能性が示唆される所見を得たので報告する。

A 64 year-old man who showed heart block from second degree A-V Block to complete heart block is reported. In this case, 10 polars catheter with 5 pairs of close bipolar electrode was used to analyse this arrhythmias. The His bundle electrogram showed split His bundle potentials. In the proximal His lead, H₁ following A wave and H₂ (proximal H₂) preceding each V wave are observed. AH₁ interval and H₂V interval are 65 msec, 45 msec, respectively.
