高度肺動脈狭窄症における右室流出路狭窄の合併について

Outflow Tract Stenosis of the Right Ventricle in Severe Pulmonary Valvular Stenosis

　高度の肺動脈弁狭窄症では二次的に肥厚した右室心筋が右室流出路の閉塞機構としてはたらくことがある^1）−4）。しかし，術前診断の過程でこのような流出路閉塞の機構は必ずしも明らかにし難い面がある^1）ので手術に際して慎重な態度が望まれる。われわれは，右室収縮期圧が200mmHgを越す高度の肺動脈弁狭窄症で肥厚心筋による右室流出路狭窄を合併した3例を経験したので報告し，若干の考察を加える。

Three operated cases of pulmonary valvular stenosis with outflow tract stenosis of the right ventricle were reported. They all revealed more than 200 mmHg of right ventricular pressure. Angiocardiography reliably demonstrated dynamic type stenosis of the outflow tract. Isolated obstructive mechanism of the outflow tract due to hypertrophy of the cardiac muscle was confirmed at operation.
