運動負荷による左右別肺機能について

Exercise-test in the bronchospirometric Method.

1．緒　言

　気管支カテーテルを用いて左右別に肺機能を調べる事は近来広く行われる様になり，特に胸部手術適応決定の為に重要視されている。この方法によつて測定される肺容積分劃から肺の換気作用とガス交換の一部を知る事が出来，呼気分析はガス交換作用の一面をうかゞう事を可能にする。従来左右別肺活量に比して酸素消費量の測定は臨床的に実用化されていない。その理由はカテーテル挿入が（1）気道を変化させ，換気作用に影響を与える事，（2）物理的刺戟として安静状態を乱し，呼吸状態を変化させ，多くの場合酸素消費量が数10％増加する事，（3）且つそれは個人差が甚しいこと等のために，一定の代謝における酸素消費量を知る事は誠に困難であるからである。

　そこで我々は運動を負荷することによつて，エネルギー代謝を一定の高さに引上げ，そのレベルに於ける酸素摂取，炭酸ガス排出の状況を求めんとしたのであり，一方これは一定の運動に対する患者の反応態度から，胸部手術適応決定の或る因子を捕えようとしたものである。

In case of the bronchospirometry using catheter, it is difficult to measure exactly the O₂-consumption under normal condition. Because the respiratory condition is changed by intubation, and the individual difference of the change of O₂-consumption is exceed. Then we intend what raise the metabolism up to some level by exercise and try to demand the state of O₂-consumption under some level. Otherwise we want to catch some factors to decide indication of thoracic operation.
