右脚ブロツクのSubtypeについて

On the Subtype of The Right Bundle Branch Block

　右脚ブロツクにみられるいろいろな型が何にもとづいているかを検討するため，最近経験した40例の分析を行つた。

　右脚ブロツクの分類法としては，Bayleyその他の方法^{1）2）3）4）5）}があるが，いずれも標準肢誘導における波形にもとづく分類であつて，かつ之等の方法の間には大差がない。それゆえ我々はとりあえずLepeschkinがBayleyの方法をmodifyした分類法^5）を採用した。第1表の左半はこの分類をシエーマにしたもので，第Ⅰ型の特徴はSⅠ＞RⅠなるもの，第Ⅱ・Ⅲ型はRⅠ＞SⅠにしてそのうちQRSⅢ上向きが第Ⅱ型，下向きが第Ⅲ型である。第Ⅳ型は高く狭いRⅠ，Ⅱと広いSⅠ，Ⅱ，Ⅲを特徴とし，第Ⅴ型はRⅠが相当巾広く，SⅠの巾に比敵するものである。第Ⅳ・Ⅴ型は更にa, bに細分される。即ちⅣの中でmean electrical axisが完全に逆転せるものを特にⅣ_bとしⅤ_aはRⅠ・SⅠが低棘高でSⅡ，Ⅲが深いもの，Ⅴ_bはmean QRS axisが左偏しRⅠが高くSⅢが深いものを云う。

Fourty cases of the right bundle branchblock were classified according to Lepeschkin'sview and their electrical positions were decided.

Most of them belonged to the Ⅰ, Ⅱ, or Ⅲ typeand the majority of the Ⅱ type was decided asvertical and the majority of the Ⅲ type ashorizontal. Therefore is was supposed thatthe cardiac position influences on the type ofthe bundle branch block.
