 Tumors with a gastric phenotype, in addition to those with an intestinal phenotype, occur in the duodenum. Gastric-type tumors are presumably derived from gastric foveolar metaplasia and heterotopic gastric mucosa, which are reactive and congenital lesions, respectively. However, a subset of these non-dysplastic lesions harbors GNAS and/or KRAS mutations ; thus, they are actually proliferative lesions associated with genetic alterations. Furthermore, the common presence of GNAS and KRAS mutations in pyloric gland adenoma and gastric-type adenocarcinoma supports the preneoplastic nature of gastric foveolar metaplasia and heterotopic gastric mucosa. GNAS mutations are molecular features of gastric-type duodenal tumors because they are virtually absent in intestinal-type tumors.
