炎症性腸疾患の画像所見と鑑別診断—敷石像，炎症性ポリポーシス，多発隆起

Differential Diagnosis in Inflammatory Bowel Disease Based on Endoscopic Findings:Cobblestone Appearance, Inflammatory Polyposis, Multiple Elevated Lesions

要旨●潰瘍性大腸炎やCrohn病は炎症性腸疾患と総称され，潰瘍形成と組織再生を慢性的に繰り返すことによって，多彩な内視鏡所見を呈し，診断に苦慮する症例も少なくない．本稿では，敷石像・炎症性ポリポーシス・多発隆起などの特徴的な所見に焦点を当て，炎症性腸疾患の鑑別診断について解説する．敷石像，炎症性ポリポーシス，多発隆起はいずれも背景粘膜の炎症や潰瘍，浮腫により形成される非腫瘍性の変化であり，隆起部分のみにとらわれることなく，背景粘膜の炎症や併存する縦走潰瘍の存在などの腸管全体像を正確に把握することが鑑別診断のポイントとなる．

　Ulcerative colitis and Crohn's disease are chronic IBDs（inflammatory bowel diseases）characterized by recurrence and the regeneration of ulcerations. IBDs present various endoscopic findings, which makes differential diagnosis difficult. This review focuses on the characteristic findings in IBDs, including a cobblestone appearance, inflammatory polyposis, and multiple elevated lesions. These lesions are non-neoplastic changes caused by inflammation and edema in the intestine. Understanding the specific findings in the different sections of the intestine, as well as the elevated areas, is the key to the differential diagnosis of IBDs.
