まれな大腸悪性腫瘍の臨床統計

Statistics of Rare Colorectal Malignancies

要旨●本邦におけるまれな大腸悪性腫瘍の現状を把握することを目的として，大腸癌研究会が実施している大腸癌全国登録に登録された症例の組織型と臨床病理学的因子および予後を調査した．1995〜2004年に登録された組織型が判明している全悪性腫瘍51,575例のうち非上皮性腫瘍は0.2％（117例）のみであり，上皮性腫瘍の中では高分化腺癌（45.7％）と中分化腺癌（45.3％）を併せたいわゆる分化型腺癌が大部分を占め，腺癌以外の悪性上皮性腫瘍は0.5％未満（246例）の頻度であった．これら登録件数が少ない腫瘍のうち，カルチノイドを含む内分泌腫瘍と類基底細胞癌を除く上皮性悪性腫瘍を対象として臨床病理学的特徴と予後を概観した．

　To clarify the clinical and pathological characteristics and prognosis of rare colorectal malignancies in the Japanese population, we investigated individual patient data between 1995 and 2004 using the JSCCR registry. The vast majority of colorectal malignancies consisted of epithelial tumors. Among them, well to moderately differentiated adenocarcinoma accounted for 91.0% ; it was followed by poorly differentiated adenocarcinoma（3.6%）and mucinous carcinoma（3.1%）. Proportions of signet ring cell carcinoma, squamous and adenosquamous carcinoma, and undifferentiated carcinoma, were less than 1%. In general, these rare malignancies presented at late stages, and their prognoses were poorer than the common histologic types.
