食道原発燕麦細胞癌の1剖検例

An Autopsy Case of Oat-cell Carcinoma Arising from Esophagus

　燕麦細胞癌は大多数が肺にみられ，カッシング症候群や種々の内分泌異常症状を伴う，いわゆるfunctioning tumorとして注目されている．今回，われわれは食道原発の燕麦細胞癌を経験したので，その臨床症状および組織学的所見について報告し，若干の文献的考察を加えた．

症例

　患　者：71歳　男性

　主　訴：嚥下困難

　既往歴：約10年陞前より1日5合は飲んだという大酒家であった．1969年12月脳卒中に罹患し，以来リハビリテーション等の治療をうけていた．熱いものは特に好まず，タバコも吸わない．

　家族歴：脳卒中の多い家系である以外は特記すべきことなし．

　現病歴：1975年5月頃より嚥下困難，体重減少が出現したために，精査のため当科受診し，食道バリウム検査にて下部食道に陰影欠損を認め直ちに入院となる．咳漱，喀疾は訴えていない．

　入院時現症：体格中等大，栄養不良，眼瞼結膜に貧血なく，眼球結膜黄疸なく，表在リンパ節触れず，胸部は心濁音界正常で肺野には理学的に異常所見なく，腹部には圧痛なく，肝脾は触知されない．言語障害および左上下肢に痙性麻痺が認められた．

　An autopsy case of a 71-year male with oat-cell carcinoma arising from esophagus is presented. The patient suffering from cerebral vascular apoplexy for the past 5 years was admitted with a complaint of disphagia and treated under a diagnosis of esophagus tumor as confirmed by X-ray, endoscopy and the result of biopsy of the tumor in our hospital. Post mortem examination revealed oat-cell carcinoma arising from esophagus.

　The cancer cell was thought to have no function, because Cushing's syndrome in his all clinical course and neurosecretary type granule by electron microscopy were not shown.
