 A case of phytobezoar caused by continuous ingestion of “Confrey” powder is reported. It was successfully extracted perorally by means of scope for biopsy.

 The bezoar, 5.7 cm long and 2.6cm wide with the shape like an eggplant, weighed 18 gr. At microscopic examination, residues of vegetable fiber bundles were found in it along with many starch grains and networks of cellulose fibers.

 While the site of the bezeoar was made sure by the fiberscope for biopsy, a small loop of silk string previously prepared was introduced down into the gastric lumen; the loop was drawn toward the bezoar by the forceps of the fiberscope and then it was made fast by the loop around it. After the fiberscope was drawn out, the string was slowly pulled up with the bezoar fast secured within the loop.

 It has been pointed out elsewhere that one can break a bezoar into fragments by the fiberscope forceps, but in most instances bezoars are not only very slippery on account of the gastric mucus over their surface, but they are also hard to make fast because they move around almost freely within the gastric lumen. Furthermore, the curets of the fiberscope for biopsy currently employed are so small that only a tiny fragment can be extracted at one time. This procedure is often very troublesome. Peroral extraction is much easier hen a bezoar has grown to certain dimensions as in this case.
