6歳男児にみられた結腸sm癌の1例と本邦小児結腸癌報告例の検討

Signet-ring Cell Carcinoma of the Colon: Report of a Case in a 6-year-old Boy

要旨　下血を主訴とする6歳，男児の下行結腸にみられた単発性有茎性病変に対しポリペクトミーを行い，組織学的にsm印環細胞癌と診断されたため，左半結腸切除，R₃郭清を行った（P₀，H₀，sm，ly₁(＋），v₀，n₁(＋），Stage Ⅲ）．術後7年再発なく長期生存が得られたので報告すると共に，自験例を含めた本邦小児結腸癌報告例57例を集計し，臨床的検討を加えた．組織学的に，約50％が粘液癌・印環細胞癌で，自験例のごとく有茎性の発育を示した印環細胞癌は認められず非常にまれであった．予後は成人に比し不良で，5年以上の生存例は自験例を含めわずか6例にすぎない．その要因として，小児結腸癌はまれな疾患で症状発現に乏しく，組織学的悪性度が極めて高いことが挙げられる．

　A 6 year-boy was referred to our hospital because of melena. Barium enema examination disclosed a polypoid lesion in the descending colon. Polypectomy was performed. Microscopic examination revealed signetring cells extending to the submucosal layer (Ⅰp type). Subsequently, he underwent left hemicolectomy (R₃). A metastatic lesion was detected in one of epiploic lymph nodes (H₀, P₀, sm, ly₁(+), v₀, n₁(+), StageⅢ, Dukes C Stage). He has had no recurrence in the 7 years since operation.
