腸炎と腸癌の合併

Crohn's Disease of the Colon Complicated by Colon Cancer

　一般に長く経過した腸の炎症性疾患において癌が発生し易いことはよく知られている¹⁾．

　とくに潰瘍性大腸炎では，10年以上経過した症例においてはかなり高率に，しかも一般の癌年齢より若年者に結腸癌の合併がみられる²⁾．欧米では予防手術すらも行なわれることがある³⁾．一方潰瘍性大腸炎と並んで代表的な慢性経過をとる非特異性炎症性腸炎であるクローン病（限局性腸炎）については，癌の合併した症例が稀である．Ammann⁴⁾はむしろ一般人口における発生率より低く，クローン病は前癌状態としては評価できないといっている．

　A case of Crohn's disease of the colon complicated by colon cancer is presented. Only a few such cases could be found in the literature. But its reports are on the gradual increase recently. In our case the diagnosis was correctly made preoperatively by fluoroscopic and fibercolonoscopic examination. We collected as many cases as possible from the literature and studied the relation between Crohn's disease and intestinal cancer.
