肝内結石症の新病型分類

Classification of Hepatolithiasis

要旨　現在まで肝内結石症の分類案は20種以上に及び，討論の場において共通の理解を得るのに多大の支障があった．今回，厚生省特定疾患肝内胆管障害調査研究班において新しい肝内結石症分類案が提唱され，大方の賛同を得ているので，ここにその解説を試みた．肝内結石症の病態を記述するため，その成因因子のうち胆管狭窄と拡張を取り上げ，結石の種類，所在，および既往手術術式を記号化し，共通の理解およびコンピューター化に便利ならしめた．

　The numerous classifications on hepatolithiasis so far proposed exceeding twenty have hindered rather than helped in reaching the common understanding of the disease. A new classification was proposed by Research Group for the Study of Damage to Intrahepatic Bile Duct and was presented here. The grade and site of the stricture and dilatation of the bile duct and the presence and kind of gallstones were coded as well as the biliary tract structures. Operative procedures were assigned a letter alphabetically and attached at the end of the coding. The classification thus proposed offers several advantages, i. e., it is solely based on the exact description of the prevailing pathology and therefore devoid of subjective bias, easy to understand and offers another advantage of easier compilation of the data which can be stored in a computer for later retrieval and computation.
