Ⅱa型Barrett食道癌の1例

Barrett's Esophageal Cancer, Report of a Case

要旨　Barrett食道に発生した多発（二重複）腺癌の1例を経験した．患者は51歳，男性．胃検診で食道の隆起性病変を指摘された．裂孔ヘルニアを合併し，食道胃入口部から口側3.5cmまでの粘膜はBarrett上皮で置換され，同上皮部に2個の癌巣が存在していた．臨床的に発見された主癌巣はⅠ＋Ⅱa＋Ⅱc，tub1，sm2，30×28mm，組織学的に偶然発見された副癌巣はⅡb，tub1，m，20×15mmであった．主癌巣は食道下部（Ei）の前壁から左側壁（1/2周），副癌巣は腹部食道（Ea）の後壁に存在していた．比較的まれな症例と思われたので若干の文献考察を加えて報告した．

　On the occasion of a mass screaning examination of the esophauo-stomach, an elevated lesion was detected in the esophagus of a 51-year-old man. The epithelium on the oral side extending 3.5 cm from the EG-junction had changed to Barrett's mucous and a double adenocarcinoma had developed on the Barrett's esophagus.

　The clinical findings showed that the main lesion was Ⅰ＋Ⅱa＋Ⅱc, tub1, sm2, 30×28 mm, Ei. The histological findings showed that the accessory lesion was Ⅱb, tub1, m, 20×15 mm, Ea.
