 The procedure of EMR(endoscopic mucosal resection)is not merely a technical matter, but a series of complicated steps including precise endoscopic diagnosis, appropriate strategy for treatment, handling of the resected specimen, and finally pathological diagnosis. It should be most carefully conducted in colorectal lesions suspected to involve the submucosal layer. An endoscopically evident submucosal cancer is usually deeply invasive, and therefore should be treated surgically. A lesion suspected to be a slightly invasive cancer can be treated endoscopically, but should be resected in one piece. En bloc resection using a snare has been carried out in lesions less than 20mm in diameter, but now can be performed in those up to around 30mm. Precise prediction of cancer depth is, of course, a prerequisite for EMR treatment of early colorectal cancer.
