家族性大腸腺腫症における陥凹型腫瘍―臨床の立場から

The Depressed Type Neoplasms in Patients with Familial Adenomatous Polyposis

要旨　家族性大腸腺腫症（familial adenomatous polyposis ; FAP）は大腸に腺腫が多発する常染色体優性遺伝疾患である．若年での発癌につながりadenoma carcinoma sequenceの発癌経路を代表する疾患である．しかし，一方で古くから陥凹型早期癌が発生することも知られているにもかかわらず，実際にその頻度がどの程度であるのかについて言及されたものは極めて少ない．陥凹型腫瘍はその生物学的悪性度は高いと考えられ，それがFAPにおいても観察されること，その頻度は決して低いものではないことを念頭に置き，拡大観察を含めた詳細な内視鏡検査による注意深い観察が不可欠であると考えられた．今後，他施設での症例の蓄積とAPCを含めた遺伝子解析が必要であると考えられた．

　It is known that not only polypoid lesions but also flat-and depressed-lesions are found in FAP（familial adenomatous polyposis）. Depressed type lesions are characteristic and important for their rapid growth and highly malignant nature. Careful observation with magnifying chromoendscopy is indispensable.

　We report the present conditions in our institution this time.
