Helicobacter pylori感染の進展と胃粘膜NBI拡大観察

Magnifying Endoscopic Findings, Using NBI, of Chronic Gastritis due to Helicobacter pylori Infection

要旨　H. pylori感染の進展による胃粘膜拡大像の変化はA-B分類で表現されている．この分類を基本にH. pylori胃炎進展のNBI併用拡大像をまとめた．非感染胃体部粘膜拡大像は集合細静脈，真性毛細血管のネットワーク，ピンホール状の腺開口部から形成されている（B-0型）．感染粘膜では集合細静脈は見えず，毛細血管のネットワークと円形の腺開口部から成るB-1型，楕円形腺開口部と胃小溝から成るB-2型，さらに開大した卵円形やスリット状の腺開口部と胃小溝から成るB-3型，さらに萎縮が進展すると溝状，スリット状の腺開口部に伴い管状・うろこ状の粘膜模様を呈するA-1型，腸上皮化生が高率に発生する粘膜では絨毛状・顆粒状模様を呈するA-2型に進展していく．この構造変化は腺開口部の変化を反映している．すなわち炎症を伴うと小円形の腺開口部から開大した腺開口部・胃小溝へと変化し，萎縮が高度になると胃小溝が連続して粘膜模様は管状・うろこ状に観察される．さらに腸上皮化生を合併すると連続した胃小溝はより深い溝となって乳頭状構造が絨毛状・顆粒状の粘膜模様として観察される．

　The magnified view of gastric mucosa changes according to development of gastritis. These magnified views are expressed in A-B classification. Magnified view of H. pylori-negative normal mucosa of the corpus consisted of collecting venules, the network of true capillaries and pinhole-pits（Type B-0）. Magnified views of gastritis of the corpus are divided into three types : type B-1 consists of round pits and a network of capillaries, type B-2 consists of oval pits and sulci and B-3 consists of dilated oval pits and sulci. Atrophic mucosa and pyloric mucosa of H. pylori-induced gastritis are divided into two types : type A-1 consists of tubular and scale-like pattern and type A-2 consists of villi and granular pattern.
