要旨　大腸M・SM癌の治療における「大腸癌治療ガイドライン第1版」の有用性と問題点に関する私見，ならびに新しく出版された第2版の改訂のポイントについて概説した．2005年に出版された「大腸癌治療ガイドライン第1版」において，追加外科切除を考慮する基準の1つとして，pSM垂直浸潤距離1,000μm以上と，SM浸潤度の基準が明確化されたことで，大腸SM癌の内視鏡治療後の方針を明確に患者に提示することが可能となり，大腸SM癌に対するover surgeryの減少に寄与している．一方，運用上の問題点として，“pSM垂直浸潤距離1,000μm以上”が“追加外科切除を考慮する”ではなく，“追加外科切除の基準である”と誤解されている可能性が危惧される．新しく出版される「大腸癌治療ガイドライン第2版」の“内視鏡治療”の変更点として，“リンパ節郭清を伴う腸切除を考慮する”条件に，① 第1版の病理学的条件に簇出のgrade 2/3が加わった．② 重要な項目に関して詳しい説明が加えられた“CQ”が登場した．CQ-1は“内視鏡摘除後の追加治療の適応基準”について，CQ-2は“最大径2cm以上のcM・cSM癌の内視鏡的摘除治療方針”について述べられたものであり，これらの大腸M・SM癌の内視鏡治療上，重要な項目について詳細な解説が加えられている．

　Personal opinions about the usefulness and the problems involved in the 1st edition of “Therapeutic guideline for the colorectal carcinomas” were described and revised outlines in a brand new 2nd edition were also described. Since one of the criteria to be considered for additional surgery after endoscopic resection for SM carcinomas stated in the 1st edition of Therapeutic guideline for the colorectal carcinomas published in 1995 was that if SM invasion distance is 1,000μm or more, therapeutic strategy for the SM cancer after endoscopic resection can be clearly recommended for patients. Consequently, surgery for SM carcinomas after endoscopic resection has been reduced. SM invasion distance of 1,000μm or more was misunderstood as a criterion for additional surgery not as a criterion for consideration about the need for additional surgery after endoscopic resection. The revised outlines about‘Endoscopic treatment’in the new 2nd edition are as follows, 1. The sprouting grade 2/3 was added a consideration criterion for additional surgery after endpscopic resection for SM carcinomas. 2. CQ appeared as a detailed explanation for the important issues concerning this problem. CQ1 is an explanation about‘Criteria for additional surgery after endoscopic resection for colorectal carcinomas’and CQ2 is about‘Therapeutic strategy for endoscopic resection of colorectal cM and cSM carcinomas with a size of 20mm or more’.
