4）早期胃癌のリンパ節転移

Lymph Node Metastasis in Early Gastric Cancer

要旨　早期胃癌に対する治療法は，内視鏡治療や縮小手術，腹腔鏡治療などの進歩により多彩となってきたが，リンパ節転移の診断に関する知見は，その治療法の選択や安全性において最も重要な指針の1つである．リンパ節転移は，早期胃癌においても頻度が低くないことから，安易なリンパ節郭清の省略や縮小は，再発の危険性を伴う．また，臨床的意義については議論のあるところであるが，現段階では微小転移の存在も無視できない．早期胃癌におけるsentinel nodeの術中迅速診断を，安全性の担保とした縮小手術も行われつつある昨今，微小転移も含めたさらに正確な病理診断が求められている．

　Pathological findings of the lymph node are very important for decision regarding therapy such as endoscopic mucosal resection and laparoscopic surgery in early gastric cancer. The reducation of lymph node dissection is attended with the risk of recurrence, because early gastric cancer with lymph node metastasis is not rare. Although lymph node metastasis is one of the important prognostic factors, the clinical significance of micrometastasis remains controversial. Recently minimally invasive surgery with sentinel node navigation surgery is performed based on the results of intraoperative pathological diagnosis in sentinel nodes. More accurate pathological diagnosis including micrometastasis is needed for early gastric cancer.
