 There are not a few patients with severe intractable ulcerative colitis (UC) who are not able to achieve improvement in spite of steroid administration in large quantities. Cyclosporine (CsA) has been proved to be highly efficacious in inducing remission in such patients with severe intractable UC and in whom total-colectomy is contemplated. Furthermore, it seems to be important to start administration of CsA in patients with this condition as soon as possible, for achieving greater improvement in QOL. Early CsA administration requires prediction of refractoriness to steroid therapy, and colonoscopic findings seem to be important for making this prediction. Early CsA-therapy seems to induce a longer time of remission than the conventional CsA therapy reported previously.
