要旨 近年ではズーム式拡大内視鏡の登場により,通常観察から瞬時に拡大観察(80倍)することが容易となり,鮮明な画像が得られるようになった.十二指腸や小腸の拡大観察では絨毛形態の変化に注目することが重要である.そのためには健常人における絨毛形態を理解することが不可欠である.ことに絨毛の萎縮や消失,胃型上皮化生の出現に注目すべきと考えられる.さらにNBI(narrow band imaging;狭帯域光観察)の導入により従来からの粘膜表面構造に加え粘膜表層の毛細血管を明瞭に観察することが可能となり,新たな知見の集積が期待される.
Recently, it has became easy to zoom from ordinary to80X magnified views in routine examination instantly, and a clear image can be provided. It is important that we pay attention to a change of villous shape observed by magnifying endoscopic observation of the duodenum and small intestine. Therefore it is essential to understand villous shapes in a normal subject. We should especially pay attention to atrophy and elimination of villi and appearance of gastric epithelial metaplasia. Furthermore, the narrow band imaging (NBI) is a newly developed device which supplies information about the microvascular architecture of the villi. That observation with magnified NBI creates new situation to disease state elucidation of a disorder in duodenum and small intestine can be expected.
