Diagnosis and Treatment for Gastrointestinal Mesenchymal Tumor

要旨　胃・十二指腸の平滑筋性腫瘍，神経性腫瘍，GISTの診断と治療について概説した．間葉系腫瘍の診断に用いるmodalityとしては，内視鏡，X線，超音波内視鏡，CT，MRI，血管造影などがある．3者を厳密に臨床的に鑑別することは困難であるが，それぞれの検査所見の特徴を理解し，総合的に診断を行うことが重要である．さらに臨床的に重要なことは，良悪性の鑑別であり，特に腫瘍径2～5cmのものでは，各種画像診断，EUS-FNAを含めた組織診断を駆使し，悪性が示唆される場合は，時期を逸することなく手術を行うことが必要である．

　We spoke of diagnosis and therapy of gastointestinal mesenchymal tumor of the stomach and duodenum. We have means to diagnose gastointestinal mesenchymal tumor such as endoscopy, X-ray, endoscopic ultrasonography, CT, MRI, and angiography. Though it is difficult to differentially diagnose gastointestinal mesenchymal tumor, we have to diagnose it synthetically using these means. On the other hand, it is also important to differentiate benign tumor from malignant tumor. Gastrointestinal mesenchymal tumor which is2～5cm in size is suspected to be malignant, so we have to treat it with laparoscopy on otter surgical methods.
