 The aim of this study was to clarify macroscopic findings of small-sized (10mm or less in diameter) gastric carcinomas from the viewpoint of histopathological features. We studied 460 lesions from 426 cases of small-sized gastric carcinomas, including 215 lesions from 194 cases of minute (5mm or less) carcinomas.

 Differentiated type adenocarcinomas were observed in 85.7 % of all cases and appeared brownish in color (76.6 %) and with an area-like pattern on their surfaces (94.2%). 51.3% of differentiated carcinomas were high-grade ones which were characterized in enhancement by their brownish surface color. On the other hand, poorly differentiated adenocarcinomas demonstrated unique findings such as discolored surface (87.2%), clear outline (87.2%), formation of islands (48.7 %), and destruction of marginal mucosa.

 As for mucous phenotype, gastric phenotype carcinomas revealed a significantly higher incidence of discolored surface (43.2%) than those with the intestinal phenotype (16.3%) (p<0.01). Carcinomas with the gastric phenotype also demonstrated a significantly higher incidence of unclear outline (54.5%) as compared with that of the intestinal phenotype carcinomas (24.5%) (p<0.01). It was also found that mucosal heights measured under a microscope revealed that the shallowest depressions were associated with gastric phenotype lesions (0.34±0.09mm) rather than with the gastrointestinal phenotype (0.48±0.11mm) and the intestinal phenotype (0.47±0.09mm) (p<0.01).
