Endoscopic Hemostasis for Gastrointestinal Bleeding―Clipping Method

要旨　消化管出血に対する内視鏡的止血術は広く普及しており，その方法も多岐にわたる．その1つであるクリップ法に用いるクリップ装置とクリップは種々のものが市販されており，止血を行う病変の部位や硬さによって使い分けることが可能である．クリップ法による止血術は，露出血管を伴う潰瘍出血や憩室出血が良い適応である．クリップ法には出血点の確認が重要であり，確実な止血のためには，可能な限り出血点を正面視し，処置を行うことが肝要である．クリップ法などの機械的止血法は組織の変性がほとんどなく，比較的安全に施行することができるが，クリップ法による止血術が困難な場合には，他法の選択や他法と併用することが重要である．

　Endoscopic hemostasis for gastrointestinal bleeding is popular now and various methods have been worked out. The clipping method of endoscpic hemostasis, is efficacious for bleeding from ulcers with exposed vessels and diverticulum. Obtaining the frontal view of the bleeding point is very important when the clipping method is performed. Because the clipping method stops bleeding by grasping vessels, it is safer than the local injection method and the coagulation method. However, if it is difficult to perform the clipping method, we should not hesitate to use other methods.
