 Based on detailed observation, such as magnification prior to endoscopic treatment, we can choose several treatment methods (EMR including piecemeal resection, ESD and surgery). Concretely, adenomatous large LST-G can be cured by EMR. ESD should be applied to large LST-NG (especially the pseudo-depressed type) and mucosal carcinoma with fibrous change showing non-lifting sign. However, safety and standardization of colorectal ESD have not been yet established. Endoscopists should select an adequate therapeutic strategy for large LST with careful consideration of their skill level.
