はじめに

　上顎洞の下壁は，歯槽突起の底を形成しており，時に根尖端が上顎洞に露出することがある^1）。歯科治療に関係する異物としては，上顎歯の抜歯時に歯根が上顎洞内に迷入する場合が最も多く^2），特に第1大臼歯は圧倒的に多いと報告されている^3）。近年施行されるようになったEndoscopic Modified Medial Maxillectomy（EMMM）は，上顎洞に対する術式であり，下鼻甲介や上顎洞内側壁，鼻涙管といった鼻腔形態を温存しつつ，上顎洞後壁や外側壁，前壁にもアプローチが可能であり，従来の内視鏡下鼻副鼻腔手術（Endoscopic Sinus Surgery：ESS）よりも広範囲の部位を処置できる^4）。今回われわれは，上顎洞内に迷入した歯根に対し，EMMMを行い摘出した1例を経験したので，若干の文献的考察を加え報告する。

　Endoscopic Modified Medial Maxillectomy（EMMM） is a technique for the maxillary sinus that can approach the posterior, lateral, and anterior walls of the maxillary sinus while preserving the nasal morphology, such as the inferior turbinate and the medial wall of the maxillary sinus. The EMMM is also useful for maxillary sinus foreign bodies.
