Abstract Purpose:We report a case of ectopic bleb formation following Preserflo MicroShunt(PFM) implantation.

Case:A 69-year-old man with open-angle glaucoma underwent PFM implantation in the left eye. Twenty-five days postoperatively, needling revision was performed for poor bleb formation. Aqueous leakage from the puncture site occurred the following day and persisted despite conjunctival suturing. An ectopic bleb developed inferotemporally along with corneal dellen. Surgical bleb reconstruction with mitomycin C was performed 42 days after the initial surgery, re-establishing normal aqueous outflow. Both the ectopic bleb and corneal dellen resolved, and intraocular pressure stabilized at 9 mmHg without medication.

Conclusion:This case suggests that conjunctival damage and aqueous misdirection after needling may induce ectopic bleb formation. Careful site selection and intraoperative precautions are important when needling is performed after PFM implantation.
