　術後5—フルオロウラシルを使用した線維柱帯切除術後に濾過胞が消失，あるいは限局して眼圧再上昇をきたした25例25眼に対し，マイトマイシンCを併用した濾過胞再建術を施行した。対象の術前眼圧は20〜35mmHg （23.8±4.3）であった。18眼（72％）の症例で無治療で眼圧が20mmHg以下となり，最終眼圧は11.6±4.4mmHgであった。残る7眼のうち5眼は点眼療法により眼圧コントロールが得られた。生命表法解析を行った結果，術後1,6,12か月の無治療での眼圧コントロール率はそれぞれ88.0±6.5,72.0±9.0,72.0±9.0％であった。術後合併症は角膜上皮障害1眼，結膜創離開1眼，前房形成不全3眼，脈絡膜剥離3眼であった。

　マイトマイシンC併用濾過胞再建術は新たな結膜切開を加えることなく良好な術後成績が得られることから，有用な術式と考えられた。

We performed surgical repair of filtering bleb with subconjunctival mitomycin C in 25 glaucoma eyes after failure of previous trabeclectomy with subconjunctival 5-fluorouracil. Preoperative intraocular pressure (IOP) under maximum tolerable medication ranged 20 to 35mmHg, mean 23.8mmHg. Follow-up period ranged 5 to 12 months, average 7.6 months. After surgery, the IOP was controlled below 20mmHg without medication in 18 eyes, 76%. The IOP in these eyes averaged 11.6±4.4mmHg. Additional topical medications were necessary to control the IOP in 5 of the remaining 7 eyes. The success probability assessed by life-table method was 88%, 72% and 72% at 1,6, and 12 months respectively. Postoperative complications included corneal epithelial defect 4%, conjunctival wound leak 4% and shallow or flat anterior chamber with choroidal detachment 12%. The finding suggest the usefulness of reconstruction of the filtering bleb with subconjunctival mitomycin C in cases of failed bleb.
