Abstract Purpose:To report a case of fundus albipunctatus with macular degeneration.

Case:A 69-year-old man with a lifelong history of night blindness, who had not undergone regular ophthalmic examinations, recently noticed decreased vision. His medical history included atrial fibrillation and myocardial infarction. Family history revealed a grandmother with glaucoma;there were no consanguineous marriages. His wife, son, and brother had no history of night blindness.

Findings:Corrected visual acuity was 1.0 in the right eye and 1.2 in the left eye. Fundus examination revealed bilateral macular degeneration with peripheral white dots. Fundus autofluorescence revealed mottled hypofluorescence and hyperfluorescence around the macula. Optical coherence tomography demonstrated a coarse, thinned outer macular layer with hyperreflective foci corresponding to the white spots. Optical coherence tomography angiography revealed no evidence of neovascularization. Full-field electroretinography showed reduced amplitude in the rod system response, while the cone system remained preserved;however, multifocal electroretinography demonstrated decreased central amplitudes. Following 3 hours of dark adaptation, electroretinographic responses improved. Genetic testing identified a homozygous RDH5 mutation(c.928delinsGAAG), confirming a diagnosis of fundus albipunctatus.

Conclusion:In elderly patients, fundus albipunctatus frequently coexists with macular degeneration. The presence of lifelong night blindness is critical for differentiating it from age-related macular degeneration. A definitive diagnosis requires multimodal imaging combined with genetic testing.
