Abstract Purpose:Although benign, retinal vasoproliferative tumors(RVPT) can be significantly compromised, particularly in cases of severe activity leading to retinal detachment. This report describes a case of RVPT with progressive proliferative changes that persisted despite multiple treatments, ultimately necessitating tumor resection and pathological examination.

Case:A 19-year-old male with no significant medical history presented with a one-month history of decreased vision in the left eye. He initially visited the hospital in October X.

Findings:During the initial examination, a reddish-brown tumor with extensive hard exudates and exudative retinal detachment was identified in the inferotemporal retina. Fluorescein angiography revealed substantial fluorescein leakage from the tumor, confirming a diagnosis of RVPT. However, exudative changes improved with 11 intravitreal anti-VEGF injections, two sub-Tenon triamcinolone injections, and photocoagulation by November X+2, and fibroproliferation in the posterior pole led to tractional retinal detachment. This requires vitrectomy, tumor cryotherapy, and scleral buckling. Postoperatively, the retina was reattached, but the traction gradually worsened toward the tumor. In April X+3, a second vitrectomy that included tumor resection was performed. Pathological analysis of the resected tumor revealed a predominance of astrocytes positive for glial fibrillary acidic protein and S-100 proteins, along with small blood vessels with partially hyalinized walls. After tumor resection, exudative changes and retinal traction improved.

Conclusion:Tumor resection may be a viable treatment option for patients with severe RVPT and high activity.
