Abstract Purpose:To report a case of iris arteriovenous malformation in an elderly patient.

Case:A 73-year-old man presented to Nagoya City University East Medical Center with blurred vision in his right eye and requested cataract surgery. His ophthalmological history included irregular follow-ups since his 60s and had previously undergone cataract surgery in his left eye. Examination revealed a best-corrected visual acuity(BCVA) of 1.2(decimal notation) in the left eye, intraocular pressure of 14 mmHg, and no abnormalities in the conjunctival or scleral blood vessels. The iris displayed slightly faint pigmentation, as well as tortuous and dilated vessels in the 3−6 o'clock positions. Anterior segment optical coherence tomography(AS-OCT) revealed a hyporeflective cystic cavity, while optical coherence tomography angiography(OCTA) confirmed blood flow in the corresponding region. No neovascularization was observed in the angle, and there were no abnormal findings in the retina or vitreous. Anterior-segment fluorescein angiography revealed clearly defined blood vessels in the early phases, with no fluorescence leakage observed in the late phases. Based on these findings, the patient was diagnosed with an iris arteriovenous malformation.

Conclusion:This case highlights an iris arteriovenous malformation diagnosed in an elderly age. While anterior segment angiography remains the preferred modality for differential diagnosis, AS-OCT and OCTA also serve as valuable noninvasive diagnostic modalities.
