Abstract Purpose:Here, we report a case of cataract surgery performed 49 years ago using phacoemulsification and aspiration(PEA), the first ultrasonic lens aspiration instrument in Japan, which was developed by the late Yasuharu Kuwahara and his colleagues.

Subjects and Methods:We investigated corneal endothelial cells, corneal thickness, corneal perception, and corneal shape analysis over a 25-year period from 1997 to 2022, 22 years after cataract surgery at our clinic.

Results:In both eyes, there were no significant changes in endothelial cells in the central region of the cornea. However, in the peripheral region, the density of corneal endothelial cells was significantly lower in the area where the ultrasound tip was inserted(9 o'clock in the right eye and 3 o'clock in the left eye)than in the other areas. Corneal thickness and perception were within normal limits, and corneal shape analysis showed no changes.

Conclusion:The cornea remained clear and stable for 49 years after surgery.
