術後前房出血の吸収が遷延した偽水晶体眼の症例

A case of longstanding hyphema following cataract surgery with intraocular lens implantation

　遷延化した前房出血を白内障手術後に呈した68歳女性の偽水晶体眼を1例経験した。出血は白内障術中に生じ，術中の止血は不可能であった。術後約1か月で出血は消退し，眼内レンズ前後面に嚢内血腫を形成した。その後，患者本人の希望で外科的治療は施行せず，血腫の吸収の徴候は9か月間みられなかった。全身検査を行ったが，吸収遷延の原因となるような所見はみられなかった。術後10か月目で血腫は吸収され，嚢内に膜形成を残したが，裸眼視力1.2にまで回復した。出血の原因，および遷延化した血腫が吸収された機序は不明であった。遷延化した前房出血は，長期間経過してもなお保存的療法で視力回復する例があると思われる。

A 65-year-old female underwent cataract surgery with intraocular lens implantation in her right eye. Uncontrollable hemorrhage in the anterior chamber developed intraoperatively. The hyphema resolved one month after surgery, leaving intracystic hematoma on both sides of the intraocular lens. The hematoma was not surgically treated by the patient's wish. Systemic examinations failed to identify the cause of hyphema and the persistence of hematoma. It ultimately resolved with the visual acuity of 1.2 10 months after surgery leaving membrane formation of the capsules. This case illustrates that longstanding hyphema following cataract surgery may undergo spontaneous resolution after conservative treatments.
