A 65-year-old female underwent cataract surgery with intraocular lens implantation in her right eye. Uncontrollable hemorrhage in the anterior chamber developed intraoperatively. The hyphema resolved one month after surgery, leaving intracystic hematoma on both sides of the intraocular lens. The hematoma was not surgically treated by the patient's wish. Systemic examinations failed to identify the cause of hyphema and the persistence of hematoma. It ultimately resolved with the visual acuity of 1.2 10 months after surgery leaving membrane formation of the capsules. This case illustrates that longstanding hyphema following cataract surgery may undergo spontaneous resolution after conservative treatments.
