超音波水晶体乳化吸引におけるマイクロパルスモードの優位性

Usefulness of micropulse mode during phacoemulsification-aspiration

目的：水晶体乳化吸引の実際および超音波チップ先端に気泡が発生する現象（キャビテーション）を可視化し，さらに異なる発振モードでの水晶体乳化吸引を評価すること。方法：水晶体乳化吸引装置と高速度デジタルビデオカメラを使用した。結果：キャビテーション撮影することができた。連続発振モードでは水晶体核片（チャタリングモーション）を記録でき，微小パルス発振モードでは超核片追従性（ハイパーフォローアビリティ）を映像化できた。結論：超音波チップの振動とキャビテーションを可視化することができ，連続発振モードと微小パルス発振モードに大きな違いがあることを映像として証明した。

Purpose：To record visually the actual process of phacoemulsification-aspiration（PEA）,record cavitation at the ultrasound tip,and to evaluate PEA under different ultrasound power modes. Method：This clinical and experimental study was made using a PEA unit and a high-speed digital video camera. Results：We could record cavitation at the ultrasound tip,chattering motion of lens nucleus under continuous mode,and hyper-followability under micropulse mode. Conclusion：We could visually document the vibrating motion of ultrasound tip and cavitation. There were remarkable differences between continuous and micropulse modes.
