 This article outlines the pathological characteristics of upper and lower gastrointestinal biopsies and polypectomy specimens in children, emphasizing how they differ from adults in the context of endoscopic diagnosis and treatment. Non-neoplastic diseases such as ulcerative colitis and Crohn’s disease (narrowly defined IBD), as well as monogenic IBD, which often presents in early childhood, eosinophilic gastrointestinal disorders, infectious enterocolitis, and post-hematopoietic stem cell transplantation gastrointestinal complications, are frequently challenging to differentiate based solely on histopathological features. Therefore, accurate diagnosis necessitates clinical information, including the clinical course and endoscopic findings. Although tumor-like lesions are rare in childhood, they can cause intestinal intussusception, obstruction, or perforation. Solitary polypoid lesions often include juvenile polyps, mucosal prolapse syndrome, and inflammatory polyps, with malignancy being rare. Malignant tumors are predominantly lymphomas, but the evaluation and follow-up of lesions associated with hereditary conditions are crucial.
