要旨

食道胃接合部の疾患を取り扱ううえで，食道胃接合部の定義は非常に重要である。食道胃接合部は，「食道と胃の境界線」や「食道と胃の間の領域」，「胃食道逆流の防止機能を形成するもの」など，異なる意味合いで使用されており注意が必要である。「食道と胃の境界線」として，本邦では食道下部柵状血管の下端が用いられているが，欧米では胃の縦走ひだの口側終末部が用いられている。こうした相違点を解消するために京都国際コンセンサス会議が開催され，食道下部柵状血管の下端を食道胃接合部と定義し，下部食道柵状血管が視認困難な場合に胃の縦走ひだの口側終末部を用いることでコンセンサスが得られた。「食道と胃の間の領域」としては，本邦の食道癌や胃癌の取扱い規約では，「食道胃接合部の上下2cmの部位」としているが，欧米では「食道胃接合部の口側1cmおよび肛門側2cm以内（Siewert分類type Ⅱ）」が用いられている。一方で，京都国際コンセンサス会議では「食道胃接合部の口側1cmおよび胃側1cm以内」とされている。

　The definition of the esophagogastric junction (EGJ) is important for management of EGJ disorders. The word “EGJ” is used in different ways: for a borderline between esophagus and stomach, for a zone between esophagus and stomach and for components of preventing gastroesophageal reflux. When the EGJ is viewed as the borderline between esophagus and stomach, the definition of the EGJ is the distal end of the palisade vessels (DEPV) in Japan and the proximal end of the gastric folds (PEGF) in Western countries. To resolve these discrepancies, the Kyoto International Consensus Conference was held, and a consensus was reached to define DEPV as the EGJ, and to use PEGF when the esophageal palisade vessels are difficult to recognize. The Japanese guidelines for the management of esophageal and gastric cancers define the EGJ zone as “within 2 cm of oral and anal sides of the EGJ,” while in Western countries, “within 1 cm of oral and 2 cm of anal sides of the EGJ (Siewert classification type II)” is used. The Kyoto International Consensus Conference defined it as “within 1 cm of oral and gastric sides of the EGJ.”
