要旨

　本稿では大腸内視鏡検査における挿入技術および関連する基本知識について解説した。大腸内視鏡検査は，大腸癌やポリープの早期発見と治療を目的として行われる重要な医療手技であり，予防医療においても大きな役割を果たす。本稿では，検査前の準備，インフォームド・コンセントの取得，抗血栓療法を受けている患者への対応，腸管前処置の重要性，スコープの選択，および鎮静・鎮痛薬の使用など，検査を安全かつ効果的に行うための基本事項について述べた。大腸内視鏡の挿入技術は特にS状結腸を越える際に用いられる「軸保持短縮法」や「Right turn shortening」といった挿入技術が重要とされる。これらの技術により患者の苦痛を最小限に抑え，効率的に検査を行うことが肝要である。内視鏡医は適切な準備と各プロセスにおける安全管理を徹底し各患者の状態に応じた柔軟な対応が必要である。

Insertion techniques such as the “axial shortening method” and “right turn shortening,” which are particularly important when navigating the sigmoid colon, are emphasized. Minimizing patient discomfort while performing the procedure efficiently is crucial. Endoscopists must ensure thorough preparation and safety management throughout each process and adopt a flexible approach tailored to the condition of each patient.
