要旨

胆管カニュレーションは，胆道疾患の診断・治療のために行われるERCP関連手技の最初のステップであり，この成否が手技全体の成否に直結する。手技や機器の開発・改良に伴い，標準的な胆管カニュレーション手技は変化しており，また，標準的な手技による胆管カニュレーションが不成功，あるいは不適切な症例に対するサルベージ手技の開発もこれまでに進められてきた。本稿ではこうした胆管カニュレーション手技の歴史的な変遷と現在行われているサルベージ手技の種類，および現時点における胆管カニュレーションのアルゴリズムについて解説した。

　Biliary cannulation is the first step in ERCP-related procedures performed to diagnose and treat biliary diseases. Its success or failure is directly related to the success or failure of the entire procedure. Standard biliary cannulation techniques have changed with the development and improvement of techniques and equipment. In addition, salvage biliary cannulation techniques have been developed for cases in which standard techniques are unsuccessful or inappropriate. We describe here the historical evolution of biliary cannulation techniques, the current types of salvage techniques, and current algorithms for biliary cannulation.
