要旨

咽頭癌に対する経口手術の歴史は古く，1951年のHuetらの中咽頭側壁癌に対する拡大扁桃的手術に端を発する。その後，顕微鏡手術の発達に伴いtransoral laser microsurgeryが開発され，さらに手術支援ロボットの登場によりtransoral robotic surgeryが考案され，海外で広く普及している。一方，本邦はわが国特有のデバイスラグによりTORSの導入が大きく遅れ，またNBIの普及も相まって，咽頭癌に対するendoscopic submucosal dissection, endoscopic laryngo-pharyngeal surgery，transoral video-laryngoscopic surgeryが開発された。2020年には鏡視下咽頭悪性腫瘍手術，2022年にはロボット支援下咽頭悪性腫瘍手術が保険収載され，早期咽頭癌の標準治療として広く普及している。本邦で開発された手術は，高価な器械を要さずコスト面に優れたメイドインジャパンの術式として，海外，特にアジアで注目を集めている。

　The history of oral surgery for pharyngeal cancer dates back to 1951, when Huet et al. performed radical tonsillectomy for a case of oropharyngeal cancer. With the development of microsurgery, transoral laser microsurgery was developed, and with the advent of surgical robots, transoral robotic surgery was invented and has been widely used overseas. The introduction of TORS in Japan has lagged behind due to a device lag problem, and combined with the spread of NBI, endoscopic submucosal dissection, endoscopic laryngo-pharyngeal surgery, and transoral video-laryngoscopic surgery for pharyngeal cancer have been developed and widely introduced in Japan. In 2020 and 2022, insurance coverage for endoscopic surgery and robotic surgery for pharyngeal cancer, respectively, was approved, and these procedures are widely used as standard treatments for early-stage pharyngeal cancer. Surgery techniques developed in Japan are attracting attention overseas, especially in Asia, as cost-effective made-in-Japan procedures that do not require expensive instruments.
