はじめに

　2019年12月に第1例目が確認されてから全世界に猛威を振るった新型コロナウイルス感染症（coronavirus disease 19：COVID-19）は，日本でも例外なく多くの感染者が発生し2023年4月時点で約3,300万人の感染者，7,400人の死者を出した。その後も増減を繰り返しつつ依然として感染者は発生しており，集中治療室（intensive care unit：ICU）に入室を要する重症患者も少なからず存在している。

　COVID-19に対する治療は，呼吸療法をはじめとしてさまざまなものが検討され，各機関から随時ガイドラインや声明が発表されてきた。COVID-19は，肺障害のみならず心筋炎などの循環器領域や深部静脈血栓症などの血液・凝固領域にも異常が生じ得る全身疾患として捉えるべきものであり，特に重症症例では集中治療の役割が重要となる。集中治療領域における，呼吸療法や薬物治療の現時点でのエビデンスや推奨をまとめた。

　Coronavirus disease-19（COVID-19）has extended worldwide since the first case was confirmed in China in December 2019. Many people in Japan were infected and approximately 33 million infections and 7,400 deaths have been reported as of April 2023. Many treatments for COVID-19, including respiratory therapy for acute respiratory failure and drug therapy have been evaluated and clinical guidelines have been developed to clarify recommendations regarding COVID-19 treatment. COVID-19 is an infectious disease that should be recognized as systemic, causing not only pulmonary damage but also abnormalities in the circulatory system, such as myocarditis, and in the coagulation system, such as deep vein thrombosis. This underscores the role of intensive care, particularly for patients with severe COVID-19. This article summarizes the current evidence and recommendations for respiratory therapy and drug treatment for severe COVID-19.
