Medication Safety and Security in the Operating Room：A Narrative Review for the Perioperative Team

要旨

　危険性の高い薬剤が頻繁に投与される手術室では，投薬エラーが重篤な有害事象につながる可能性がある。また，時間的な制約から投薬の記録や情報伝達にもエラーが生じやすい。投薬エラーを防止するためには投薬プロセスの各段階におけるエラーの原因，ヒューマンエラーの心理学的要因や投薬システムに内在する潜在的な危険性を分析・把握してこれらを排除する必要がある。一方，手術室の薬剤は法令でその扱いが規制されるものが多く，その乱用・誤用を防ぐために厳格な管理が要求される。本総説では国内外の主要な文献をもとに，周術期管理チームが投薬安全や手術室の法規制薬剤の管理に取り組むために必要な基礎知識を概説する。

　In the operating room, where high-alert drugs are frequently administered, medication errors can lead to serious adverse events. In addition, due to time constraints, errors are likely to occur in the recording and information transmission processes of medications. To prevent such medication errors, understanding the types of errors at each stage of the medication process and countermeasures based on the psychological classification of human errors are required. Furthermore, the perioperative team needs to examine the potential hazards in the system of the medication process and to eliminate them. In addition for medication error prevention, strict control of legally regulated drugs, whose abuse and misuse are problematic, is required. This review outlines the basic knowledge necessary for the perioperative team to work on medication safety and management of operating room drugs based on domestic and foreign literature.
