外陰部に生じた線状基底細胞癌の1例

A Case of Linear Basal Cell Carcinoma on the Vulva

はじめに

　基底細胞癌（basal cell carcinoma：以下，BCC）は高齢者の顔面など，露光部に好発する。外陰部での発症例は少なく，その腫瘍学的性格は不明な点が多い。

　また線状基底細胞癌（以下，linear BCC）は1985年にLewis 1）が線状の皮疹を呈するBCCとして初めて報告した比較的まれな臨床型である。今回われわれは，外陰部に生じたlinear BCCの症例を経験したため，文献的考察を加え報告する。

　A 66-year-old female presented with a rash on the left side of her labia majora that had first appeared 3 years earlier. The rash was actually a 30×4-mm linear black lesion that was diagnosed as superficial basal cell carcinoma (BCC) by excisional biopsy. With the patient under local anesthesia, we excised the BCC with 5-mm surgical margins and a partial fat layer. There has been no recurrence at 1 year post-surgery. BCC usually occurs on the face of elderly individuals; the development of BCC on the genitalia is rare. Among the various BCC clinical patterns, linear BCC is an uncommon morphologic variant. It is defined as ʻa lesion that extends preferentially in one direction, resulting in a tumor or plaque with a length much greater than its width greater than 3:1 ratioʼ. There are many unclear points about the oncological character of linear BCC, and clinicians should thus become familiar with this BCC variant.
