遊離皮弁により再建した動静脈奇形切除術の長期治療経過

Long-Term Follow-Up of Resection of Arteriovenous Malformations with Reconstruction Using Free Flaps

はじめに

　動静脈奇形（arteriovenous malformation：以下，AVM）の主な治療法には，塞栓術などの血管内治療や硬化療法および切除術があるが，進行例では根治が困難であり再発しやすい 1）。巨大な病変に対しては，症例に応じた治療方法や治療時期の選択が重要である。われわれは，巨大病変に対しての血管内治療や硬化療法はあくまでも潰瘍や出血および疼痛の改善などの病勢の制御を目的とする手段であると考えている。最終的に根治を目標にすることが可能な病変であれば，外科的に病変を切除することが望ましい。しかし実際は，機能障害や整容性の問題，致死的出血のリスクなどを考慮したうえで手術を計画するため，結果的に完全な切除とならないこともしばしば経験する。

　巨大なAVMにおいて，根治を目的とした積極的な切除術は，遊離皮弁などによる組織欠損の被覆が必要となることが多い。本稿では，当科でAVM切除後に遊離皮弁により再建した症例の長期経過を報告する。

　The main treatment methods for arteriovenous malformation (AVM) include endovascular therapy such as embolization, sclerotherapy, and resection; however, it is challenging to achieve a cure in advanced cases.

　For huge AVMs, it is crucial to choose appropriate treatment methods and timing based on the individual case. Reflecting on our own case experience, resection and reconstruction with free flaps were considered suitable for AVM cases in the head and neck region. On the other hand, for AVM in the limbs, when selecting a procedure that preserves length and uses a free flap, special attention must be paid to the possibility of recurrence of the lesion.
