 Objective: We evaluated the efficacy and safety of the macrolide compound sirolimus for the treatment of refractory vascular malformations.

 Methods: Between January 2018 and December 2023, 22 patients with difficult-to-treat vascular malformations received oral sirolimus treatment at our institution. We assessed the treatmentʼs effects on hemorrhage, vesicle regression, and pain improvement. Blood trough levels were maintained at 5–15 ng/mL.

 Results: Significant improvement in hemorrhage was observed in all 22 cases. Regression of vesicles was noted in 27.2 % of the cases. Pain relief was achieved in 53 % of the patients, with notable success in limb lesions. Severe side effects included interstitial lung disease (n=1) and nephrotic syndrome (n=1), both of which resolved upon the discontinuation of sirolimus.

 Conclusion: Sirolimus shows promise for managing patients with vascular malformations, particularly for hemorrhage and pain, although it can cause serious adverse effects that require careful monitoring.
