顔面の巨大動静脈奇形に対し全摘出術と遊離皮弁による再建を行った3例

Three Cases of Giant Arteriovenous Malformation of the Face Treated with Total Resection and Reconstruction

はじめに

　動静脈奇形（arteriovenous malformation：以下，AVM）に対する治療において，塞栓術や硬化療法などの血管内治療単独では，それらの治療から潰瘍を生じると逆に大量出血などによってADLを悪化させてしまうことがある。また，手術療法では部分切除を行うと残存したAVMが急速に増大することもある。他院にて複数回の治療が行われこのような状況に陥った症例において，AVMの根治的治療を行うにはnidusといわれる部分を外科的に全摘することが必須と著者らは考え，現在まで一貫してこの治療方針に沿った治療を行ってきた 1）2）。

　今回われわれは，他院での複数回の血管内治療後に顔貌の著しい変形や視力障害および大量出血などを生じた3例の顔面巨大AVMに対し，全摘出術と遊離皮弁による再建術を行ったので，若干の文献的考察を加えて報告する。

　We report the case details of three patients with a giant arteriovenous malformation (AVM) of the face who underwent total resection and reconstruction using free flaps. For the optimal resolution of an AVM, it is important to completely remove the nidus, and the following specialists are necessary for this purpose, (1) a radiologist who can reliably perform preoperative embolization, (2) an intensive care physician who can perform ICU management after embolization, (3) a head and neck surgeon who can perform wide excisions of the head and neck, and (4) a plastic surgeon who can perform reconstruction using free flaps. Such team-based medical care is essential for treating giant AVMs of the face.
