はじめに

　動静脈奇形（arteriovenous malformation：以下，AVM）はほかの血管奇形と同様に全身のあらゆるところに認められるが，体幹部はほかの部位と比較して少ないとされる 1）。体幹部AVMの治療の詳細を記述した報告は少ないものの，皮下に限局する病変に対する外科的治療 2）や，胸壁に接する病変に対する血管内治療の報告 3）などがある。今回われわれは胸壁に接し，複数の流入動脈を有する巨大な背部AVMの治療を経験したので報告する。

　We treated a 35 -year-old female patient with a massive arteriovenous malformation (AVM) lesion in her back that caused significant intraoperative hemorrhaging. The lesion had numerous feeding arteries, and the preoperative ligation and embolization of these arteries was insufficient for the control of the intraoperative bleeding. The combined application of the muscle patch graft technique and tie-over fixation was effective in controlling the life-threatening bleeding from the chest wall and intercostal muscles.

　In the future, for massive lesions in the trunk region, a treatment plan that includes thorough endovascular treatment to control the blood flow to the lesion and a bordering method for the excision should be considered.
